Welcome to RPGAnywhere — a product of Qualeto LLC, a sprawling enterprise of exactly one human being with approximately seventeen competing personalities, the attention span of a caffeinated squirrel, and a deep, abiding love for tabletop RPGs.
There are actually forms on this page now. I resisted for a long time. I lost. The forms are below. They go into an inbox I check with enthusiastic inconsistency — somewhere between "immediately" and "several days later when I surface from a hyperfocus spiral about encounter balance."

Discord. Two servers, depending on what you need.
It's where I live. It's where I work. It's where I accidentally spend three hours debating the relative merits of different dice-rolling algorithms when I was supposed to be doing literally anything else.
